Abstrak
Kala II dalam proses persalinan jdimulai jsaat jpembukaan jserviks jtelah jmencapai jtahap jmaksimal jhingga jbayi jlahir jke jdunia. jPada jfase jini, jtubuh jibu jmembutuhkan jkerja jsama jantara jkontraksi jrahim jyang jkuat, jteknik jmengejan jyang jtepat, jserta jposisi jtubuh jyang jmendukung jturunnya jkepala jbayi. jJika jposisi jmengejan jtidak jsesuai, jibu jdapat jmengalami jkelelahan jlebih jcepat jdan jdurasi jpersalinan jbisa jmenjadi jlebih jpanjang, jterutama jpada jibu jyang jbaru jpertama jkali jmelahirkan. jStudi jini jdilakukan juntuk jmembandingkan jlama jkala jII jpada jibu jprimigravida jyang jmenggunakan jposisi jsemi jFowler jdan jposisi jlitotomi jdi jTPMB jChilyatunnisa’. jPenelitian jini jmenggunakan jpendekatan jkuantitatif jdengan jdesain jkomparatif. jSebanyak j30 jibu jmenjadi jresponden, jterdiri jdari jdua jkelompok jmasing-masing j15 jorang. jData jdikumpulkan jmelalui jobservasi jlangsung jserta jcatatan jpartograf, jkemudian jdianalisis jmenggunakan juji jIndependent jt-test jmelalui jSPSS. jHasil jmenunjukkan jbahwa jrata-rata jkala jII jpada jposisi jsemi jFowler jadalah j35,6±8,2 jmenit, jsedangkan jpada jposisi jlitotomi jmencapai j52,4±10,5 jmenit. jPerbedaan jtersebut jterbukti jsignifikan jdengan jnilai jp=0,001. jTemuan jini jmenunjukkan jbahwa jposisi jsemi jFowler jlebih jmenguntungkan jkarena jmembantu jgravitasi jbekerja jlebih jefektif, jmeningkatkan jkenyamanan jibu, jserta jmempercepat jpenurunan jkepala jjanin. jOleh jkarena jitu, jbidan jperlu jmemberikan jedukasi jterkait jpilihan jposisi jpersalinan jyang jpaling jsesuai jbagi jkondisi jibu jdan jbayi.

Abstract
The second stage of labor refers to the period starting from full cervical dilation until the baby is delivered. At this point, successful birth depends on powerful uterine contractions, synchronized pushing from the mother, and positioning that helps the baby move downward. If the pushing position is not appropriate, it can cause greater fatigue and extend labor duration, especially for first-time mothers. This research compared the length of the second stage among primigravida women using semi-Fowler and lithotomy positions at TPMB Chilyatunnisa’. A quantitative comparative design was used with 30 respondents, split evenly into two groups. Data were gathered through observation and partograph records and analyzed using an Independent t-test in SPSS. The results showed a shorter average duration in the semi-Fowler group (35.6±8.2 minutes) compared to the lithotomy group (52.4±10.5 minutes), with a significant difference (p=0.001). This indicates that semi-Fowler positioning may offer advantages by supporting gravity, increasing comfort, and assisting fetal descent. Midwives should therefore guide mothers in choosing safer and more effective delivery positions.
